You seem to be in same situation as mine. We both don’t want our child to lose the mother, we want the mother to get professional help on their mental health. However the child custody cases are so discriminating against fathers and Social Services try to play games and try to get the fathers to do their dirty work and complicate things. It is like they cannot report/write any thing against the mother. Tread very carefully with what SS tell you.
Why do you need to go back to family courts if she is on supervised contact? who is your son staying with currently? How old is your son?
Have you tried speaking to the school? You need everything in written from both SS and School. Get evidence the time he arrives at school, days he does not turn up or any other concerns that may been reported e.g dirty clothes, accidents or injuries.
Are you on speaking terms with your ex? What help has the mother received? I appreciate what you are doing by not alienating him from the mother. Fair play to you.
